18:37 to doctors that have shared information 18:38 with me that are in that setting because 18:40 I'm not doing emergency medicine now I 18:41 have him for almost 20 years I was 18:43 trained in emergency medicine in general 18:45 surgery but my practice is dealing with 18:47 cancer and other types of chronic 18:50 disease autism we have patience from 93 18:51 countries so I'm not dealing with the 18:53 acute medical situation but I have an 18:55 into many of the doctors that are taking 18:57 care of it and I'll just I'll just read 18:59 you some of the things I mean that the 19:00 doctor that from the ICU er scenario 19:03 that was in New York that whose video 19:05 and viral they shut him down they 19:06 censored the video why would you censor 19:08 a doctor that's in the field that's in 19:10 the in the trenches taking care of 19:12 people and what he's stating why are 19:14 they not covering with allowing him to 19:16 be able to make his statement you know 19:18 bottom line is this Gary when you have 19:21 an acute care situation you have a 19:23 respiratory condition the first thing 19:25 that you want to remember is to maintain 19:26 the airway so in trauma medicine and 19:28 emergency medicine we have what we call 19:30 the ABCs of medicine AAA is airway B is 19:33 breathing C circulation so if the airway 19:35 head is as a potential being compromised 19:37 you want to stabilize that airway so 19:39 howhow do you make sure their person is 19:41 able to maintain that airway we start 19:43 off easily with liters of oxygen for 19:45 nasal cannula you increase it to 5 10 19:47 whatever at that point then you can go 19:49 to the system breathing you can go to a 19:50 CPAP which is continuous positive airway 19:52 pressure then you can go to by path 19:53 which is bi-level positive airway 19:55 pressure and there's studies that show 19:56 that actually CPAP violet BiPAP is more 20:00 effective than intubation but as soon as 20:02 you start incubating a person now you've 20:04 got many other things that happen okay 20:05 you've got the Tron 20:06 the Barrow trauma from the mechanical 20:08 ventilation that's an issue you've got 20:10 aspiration pneumonia as an issue you've 20:12 got the actual intubation process that 20:14 can cause more inflammatory cascade and 20:16 trauma to the to the airway there's all 20:18 sorts of video from set you 46:30 book YouTube and Facebook but they can't 46:32 censor it from your own devices and as 46:34 gary said and i appreciate you saying 46:36 that gary get this information out into
